Medication Management

It's a process that not only focuses on the medications themselves but also on the education and empowerment of clients to manage their health proactively.

Joann Diaz, PMHNP-BC

Joann is a board-certified Psychiatric Mental Health Nurse Practitioner with a Master of Science in Nursing from Regis College, Massachusetts. With over 35 years of nursing experience, she has cared for individuals across the lifespan, including those with intellectual, behavioral, and mental disabilities. For the past 15 years, Joann has focused her clinical expertise on treating a wide range of mental health conditions, including ADHD, Generalized Anxiety Disorder, Borderline Personality Disorder, Major Depressive Disorder, Attachment Disorders, and Bipolar Disorder. Licensed in Connecticut, Joann specializes in psychiatric medication management and holds prescriptive authority through the American Nurses Credentialing Center (ANCC). She is adept at collaborating with multidisciplinary teams to develop comprehensive treatment plans, ensuring optimal patient outcomes.

Gabby Laferriere, DNP, APRN

Gabby is a board-certified Family Nurse Practitioner (FNP) with a focus on mental health. She obtained her FNP and doctorate degree (DNP) from Sacred Heart University in Fairfield, CT.
Gabby's combined experience with working in mental health and primary care allows her to utilize a holistic approach in treating “the whole person”, both mind and body.
There are many variables that impact our mental health beyond a neuro-chemical imbalance. Variables such as poor gut health, nutritional deficiencies, chronic disease, and hormonal imbalance all play a very important role in the state of our mental health. Treating both mind and body is an integral part of her treatment planning process
Gabby provides treatment across the lifespan, including children, adolescents, and adults.

Janet Black, FNP-C

Janet, (she/her), is a certified Family Nurse Practitioner with more than 20 years of nursing experience. She brings a diverse background in primary care, oncology, mental health, and gender-affirming care. Her clinical focus is on helping patients manage depression, anxiety, and related conditions, with a particular passion for serving the LGBTQ+ community by prescribing gender-affirming hormones and providing inclusive, compassionate care.

Janet is dedicated to treating each patient as a whole person with a unique story that deserves to be heard. She works to remove barriers to care, meet patients where they are, and help them find balance in an ever-changing world. She believes in empowering patients through education, ensuring they have the knowledge and confidence to take an active role in their health and advocate for themselves at every step.
